2. Return it to the practice via post or email the completed form and photographic ID (e.g. a driver's licence) to gram.scotstownadministrator@nhs.scot

3. In about 7-10 working days please check your emails. You should have received an email inviting you to create an account with vision online. If you do not yet have an email then please wait for a few more days and check again, or look in your junk inbox, just in case.

If you wish for someone else e.g. a partner, to be able to discuss your medical wellbeing on your behalf, please complete the form using the link below by printing it off, completeing and handing back to reception. This form also covers consent for methods of contact e.g. text, email, mobile number.

If you wish to rescind consent after completing this form, you may. Please write to the practice informing them of this, signing and dating this letter.
